Any ideas what’s wrong?

No, you're running Nginx Proxy Manager on TrueNAS SCALE. The latter really isn't that relevant to the issue, but the former definitely is. And unfortunately, NPM is notoriously opaque when it comes to troubleshooting cert issuance issues. Best recommendation I have is to ask on their support channels:
