It seams that I am having (exactly) the same issue with this:

My domain is:

I ran this command:
letsencrypt certonly --webroot -w /var/www/ -d -d

It produced this output:
An unexpected error occurred:
Error creating new authz :: Policy forbids issuing for name
Please see the logfiles in /var/log/letsencrypt for more details.

My web server is (include version):

The operating system my web server runs on is (include version):
Linux version 4.4.0-64-generic

I can login to a root shell on my machine (yes or no, or I don't know):

Good news - the update has been performed in production. You should be able to issue for domains without seeing any policy errors. Please let me know if you still encounter any problems.
