I have the issue described below. I would also like to mention that:
1. Port 80 is OPEN
2. Anonymous authentication is enabled on the IIS site
My domain is: udb.effectmanager.com
I ran this command: win-acme with administrator permissions -> N for new certificate -> chose domain above -> confirmed
It produced this output: Invalid response from …/.well-known…; Response type: 403
My web server is (include version): Apache
The operating system my web server runs on is (include version): Windows Server 2012 R2
I can login to a root shell on my machine (yes or no, or I don’t know): don’t know
I’m using a control panel to manage my site (no, or provide the name and version of the control panel): IIS

That's not an IIS, so your IIS configuration isn't relevant.
Works Win-Acme (never used) really with an Apache / Varnish on Windows?
You may use something like webroot, so the IIS bindings are ignored.
And that Varnish may be an additional problem.
